Step 2: Complete a Customer Set Up Form
Please complete one of the below forms. Be sure to print a copy for your records before submitting.
Customers located within the United States, please use this form:

Step 3: Complete a Customer Set Up Form
If you qualify as a Sales Tax Exempt Business in the states listed below you must provide a signed State Tax Exempt form to Great Northern Equipment to keep on file. Your account will be charged tax until we receive this form.
GNE is registered and authorized to collect and remit sales tax in the following states:
*We collect simplified sellers use tax on transaction(s) delivered into Alabama and the tax will be remitted on the customer’s behalf to the Alabama Department of Revenue. Seller’s program account number is SSU-R010053684
Please refer to the Department of Revenue for further information on sales and use tax and to obtain the proper forms. Once completed, please fax the completed form to 763-428-4821. Or you can mail it to:
